The use of the title “Doctor” refers to Dr. Lincoln’s professional credential in functional nutrition and should not be interpreted as representing licensure as a physician or other medical practitioner.

Functional Holistic Nutrition services are not a substitute for care from a licensed physician or other qualified healthcare professional.

3. CLIENT RESPONSIBILITY & MEDICAL CARE

You remain responsible for your own healthcare decisions.

You should consult your physician or other qualified healthcare provider regarding medical conditions, symptoms, medications, pregnancy or nursing, and before making significant changes to your nutrition, supplementation, exercise, lifestyle, or healthcare regimen when appropriate.

Never disregard or delay seeking professional medical care because of information obtained through this website or Functional Holistic Nutrition services.

4. INDIVIDUAL RESULTS & NO GUARANTEES

5. FUNCTIONAL LABORATORY TESTING

Functional and conventional laboratory information may be considered when appropriate as part of a broader nutrition and wellness assessment.

Laboratory testing is not automatically required for every client and is recommended selectively when additional information may meaningfully inform a client's nutrition or wellness strategy.

Unless expressly stated otherwise in the description of a particular service or program, laboratory testing and laboratory fees are separate from professional service fees.

Any laboratory interpretation provided by Functional Holistic Nutrition is offered within the context of functional nutrition education and is not a medical diagnosis.

6. SUPPLEMENTS & PROFESSIONAL-GRADE PRODUCTS

Supplement recommendations, when appropriate, are individualized and intended to complement—not replace—foundational nutrition, lifestyle, and appropriate medical care.

Supplements are generally purchased separately unless expressly included in a particular service or program.

Clients may be provided access to professional-grade products through the Functional Holistic Nutrition Clinical Apothecary or affiliated practitioner dispensaries. Purchasing recommended products through Functional Holistic Nutrition is not required unless specifically stated as part of a particular offering.

Individual responses to supplements vary. Clients should inform their qualified healthcare providers about supplements they are using, particularly when taking medications, managing medical conditions, pregnant, nursing, or preparing for medical procedures.

7. SERVICES, SCHEDULING & PAYMENT

Prices, service descriptions, program structures, and payment options are provided at the time of booking or enrollment and may be updated periodically.

Payment is due according to the terms presented for the applicable service or program.

Scheduling, rescheduling, cancellation, refund, and program-specific policies presented during booking or enrollment form part of the agreement governing that service.

Certain services may also require completion of separate intake documents, informed consent forms, practice policies, or client agreements before services begin.

8. FUNCTIONAL NUTRITION STRATEGY SESSION

The 60-Minute Functional Nutrition Strategy Session is an individualized professional consultation designed to explore a client's health concerns, goals, relevant history, current nutrition and lifestyle patterns, and potential priorities or next steps.

Booking a Strategy Session does not obligate the client to enroll in an ongoing program, nor does it guarantee acceptance into or suitability for any additional Functional Holistic Nutrition service.

If additional assessment, laboratory testing, or ongoing functional nutrition support appears appropriate, available options may be discussed with the client.

9. PRIVACY & DATA PROTECTION

Functional Holistic Nutrition respects your privacy and handles personal information in accordance with its Privacy Policy.

Information may be processed through third-party service providers necessary to operate the website and practice, including scheduling, communications, payment processing, website hosting, client-management systems, and other business technologies.
